改革开放以来,我国的卫生医疗体制发生了巨大变化。作为医疗服务主体的公立医院也经历了巨大的制度变迁,其社会角色和功能定位几经调整和变化,运营机制也随之发生了巨大的变化。实质上,公立医院已经从计划经济时期的服务大众、追求公益性社会目标的国有组织,转变成兼有追求商业利益和经济效益的准市场组织,除了继续保持国有身份外,其运营行为已经具有市场的特点且具有一定垄断性。在这种背景下,新一轮医改将公立医院改革确定为“五项重点任务”之一。从微观层面看,公立医院的发展和改革问题,直接关系到“看病难、看病贵”问题的解决;关系到医改的整体推进进程;关系到医务人员的全面发展以及公立医院的自身发展。从宏观层面看,公立医院还承担着事业单位改革探路者的角色,对探索创新社会管理的实现路径具有重要示范作用。
<<Since China’s reform and opening up,great changes have taken place in our health care system. As the main body of the medical service system,public hospitals also have experienced great changes,their social roles and function orientation go through several adjustments and changes,and their operating mechanism has taken place great changes. In essence,public hospitals have gone from state-owned organizations in the planned economy period,which serve public and pursue public welfare social goal,into the quasi-market organization pursing commercial interests and economic benefit,in addition to continue to maintain the state identity,their operating behavior already have the characteristics of the market and have a certain monopoly. In this context,the new round of reform identifies the reform of public hospitals as “One of the Five Key Tasks”. From the micro level,the development and reform of public hospitals is directly related to the solution of “the difficulty and costliness of medical service”;Related to the whole advancement process of the reform;In relation to the all-round development of the medical staff and public hospitals’ own development. From the macroscopic level,public hospitals also assume the role of pathfinder of institution reform,and play the important exemplary role in exploring the implementation path of innovation of social management.
